Fight Lab MMA LLC Hold Harmless and Liability Waiver

I {name}, have voluntarily submitted my application for registration as a student at Fight Lab MMA LLC. By submitting the application for membership I certify that I am fully aware of and understand the inherent dangers in participating in activities involving kickboxing, grappling, brazilian jiu jitsu, mixed martial arts, muay thai, boxing, wrestling, kids kickboxing, kids grappling, kids brazilian jiu jitsu, kids mixed martial arts and fitness related activities. I {name} understand and agree that the operators of Fight Lab MMA LLC, its owners, instructors, or students will not be held responsible for my safety. 

I {name} understand that participating in these activities has an inherent danger that includes but is not limited to:

concussions
broken bones
torn muscles
bruising or contusions
possible disfigurement

I {name}, understand and agree that the operators of Fight Lab MMA LLC, its owners, instructors, or any other student their agents or assign, or any other individual or entities associated with Fight Lab MMA LLC, may not be held liable in any way for any occurrence or in connection with my membership or participation on any training my result in injury, death, or any damages to myself or to my family, dependents, heirs or assigns. I understand and agree that in consideration for being allowed to be a student in this program, I hereby personally assume all risks involved in connection with the same: and furthermore, I release forever the entity associated with this program.

I further state that I am of lawful age and legally competent to sign this agreement and that my signing this agreement is my own free act (unless signed by a parent or legal guardian). I also understand and agree that the terms herein are contractual, and they are not a mere recital or simply for information purposes.

I have read, understand and am fully informed myself of the contents of this agreement. I assume my own responsibility for my physical condition and capability to perform under this program, which I may participate in.
